Today, out of nowhere I got a very bad pain from my shoulder all the way up to my ear. ?

Also, when I lay down on my left side or recline, I can hear a sloshing sound with my heartbeat. This happened back in September and I went to a heart specialist, got tests done and everything came out fine. It scares me it just happened again though. What could this be?
